Sexual Assault Reporting Forms

Relationship/Domestic Violence and Sexual Assault Data Collection Form
This form should be completed by any staff or faculty person at Penn State Altoona who aids a victim of sexual assault.

Sexual Assault Report Form
This form, as well as the form found in the Penn State Protocol for Sexual Assault, must be completed by ANY MEMBER OF THE CAMPUS SECURITY AUTHORITY. Campus Security Authority is defined as: police/security personnel, resident coordinators, resident assistants, access monitors, officials with "significant responsibility for student and campus activities," director of athletics/coaches, faculty or other advisors to student organizations and the dean of students.
